Not getting the service I signed up for

My broadband went live on 17 September and I had signed up for the promise of up to 67mb of download speed. After not receiving this and 2 engineer visits, I was told by Open Reach that the maximum I can get is 40mb due to old cables in the exchange/road. What is my next step?  

My next question is the offer of 1 years Amazon Prime free. This also has not been forthcoming. Not a great impression so far and I’m getting a service that isn’t living up to its description.  Again, what is my next step?
